Standard H15

Objective
Adjuvant chemotherapy fails to influence survival time of malignant glioma patients. Boswellic acids (BA) are putative active compounds from extract of gum resin (EGR). In a clinical study EGR was shown to reduce peritumoural oedema and subsequently improve the patients’ clinical condition. Additionally, the resin was able to inhibit tumour growth in rat glioma model and human glioblastoma (GBM) cell culture. This randomised prospective study wants to elucidate whether EGR in the formulation H15TM (4 x 23mg/kg) has proliferation-inhibiting effects in patients with malignant glioma.
Methods
The study was divided into of two arms: Sixty-eight GBM patients were included into the in vivo arm. Survival time was compared between a H15TM treatment group (n=35) and a control group (n=33). In the in vitro arm the antiproliferative effect of isolated boswellic acids purified from H15TM by high pressure liquid chromatography (HPLC) was analyzed on primary cell cultures from 7 GBM patients of this series.
Results
Treatment with and without H15TM did not show any statistical difference in the survival time. A concentration dependent inhibition of glioma cell growth was achieved with the following 3 BA fractions of the cell culture experiments: 11-keto boswellic acid, acetyl-11-keto boswellic acid and acetyl boswellic acid.
Conclusions
These data demonstrate an influence of boswellic acids on glioma growth in vitro. No effects on survival time were observed in H15TM treatment of glioblastoma patients. In comparison to the encouraging results on survival time in the rat glioma model (3 x 240mg/kg) dosage of EGR seems to be the crucial factor for effect on tumour growth in malignant brain tumours.
